The Petitioner seeks to adopt the following --male minor, namely -------------------, born on the --- day of -------, 19--, in ----------, -------- County, ----, and said minor resides with the Petitioner and the Respondent, -----------------------, at -------------------. ---------------, ------ County, Kentucky.
3. The Respondent, -----------------------, the natural mother of the minor child, was born on the ---- day of ----, 19-- in ------------, ------ County, -------------- and she has been a bona fide and continuous resident of the Commonwealth of Kentucky for ----------- years.

6. The child to be adopted has, to the best of the Petitioner's knowledge, no property of any kind or description.

9. The minor child, -------------------, to be adopted herein, has no legal guardian other than her natural mother.
10. The natural mother of the child to be adopted herein, -----------------------, and the natural father of the child to be adopted herein, ----------------, have given their consent to this Adoption and will file Consents to Adoption and Waivers of Notice and Entry of Appearance, copies of which are to be filed of record herein.
11. The Petitioner and Respondent, -------------- ----- --------, the natural mother, herein have cared for, nurtured and provided support and maintenance required for the care and upbringing of the minor child to be adopted herein since --------- --, 19--, the date of their marriage, and the best interests of the minor child would be served if the Adoption requested was granted.
12. The Petitioner, -------------------------, is desirous of adopting the minor Respondent, -------------------, as his child and he is in a financial position to adequately and properly provide for her present and future needs and if permitted by this Court to adopt h-- as his own, he would bestow upon h-- the same degree of love, care, affection and advantages which he would bestow upon a child born of his marriage.
13. Pursuant to K.R.S. 199.480(3), no Guardian Ad Litem need be appointed to represent the interests of the minor child herein.

14. The best interests of the minor Respondent will be promoted by the adoption and the minor Respondent is suitable for adoption.

CONCLUSIONS OF LAW
Based on the foregoing, a Judgment of Adoption shall be entered as follows:
