The Petitioner and Respondent, -------------- ----- --------, the natural mother, herein have cared for, nurtured and provided support and maintenance required for the care and upbringing of the minor child to be adopted herein since --------- --, 19--, the date of their marriage, and the best interests of the minor child would be served if the Adoption requested was granted.

The Petitioner, -------------------------, is desirous of adopting the minor Respondent, -------------------, as his child and he is in a financial position to adequately and properly provide for her present and future needs and if permitted by this Court to adopt h-- as his own, he would bestow upon h-- the same degree of love, care, affection and advantages which he would bestow upon a child born of his marriage.

14. The best interests of the minor Respondent will be promoted by the adoption and the minor Respondent is suitable for adoption.
